Consent forms for interviews/ interviewees

 

Here is the template I referred to when constructing my own for my interviews with my 5 person focus group. This is not something that I have ever had to do before so the experience was interesting and quite tricky. I am creating a documentary on something very sensitive especially to some people so I was sure to inform all participants that even after signing the consent form they are still able to back out of the interview/focus group. I have also made it clear that if at any point after publication, at request of participants, I will remove content that they are involved or referenced in. 

All participants will sign this consent form before being interviewed.
